Google Form QR Code Generator
A Google Form QR code opens your form the moment it's scanned — a feedback survey, an event RSVP, a lead-capture form, a class sign-up. Get the form's share link from Google Forms and paste it below, then print the code wherever you want responses.

What happens when someone scans it
The phone opens the form in the browser, ready to fill in — no app, no Google account needed unless you set the form to require one. Responses land in your linked Google Sheet as normal. The code encodes the form link, so you can edit the form's questions any time and the printed code keeps working.
How to make a Google Form QR Code
- 1
Get the form link
In Google Forms, click Send, choose the link icon, tick 'Shorten URL', and copy it.
- 2
Paste it here
Enter the link. Scanning opens the live form — edits to the questions don't break the code.
- 3
Design the code
Add a caption like 'Scan to give feedback' and keep the contrast high for print.
- 4
Download and display
SVG for print, PNG for slides and screens. Test on two phones first.
Where to use it
- Table cards and receipts asking for feedback
- Event signage for RSVPs and session sign-ups
- Classroom and training handouts
- Trade-show booths for lead capture
- Noticeboards for club and volunteer sign-ups
- Product inserts for warranty registration
Frequently asked questions
Do people need a Google account to fill in the form?
Only if you turned on 'Limit to 1 response' or restricted it to your organisation. By default anyone with the link can respond anonymously.
Can I edit the form after printing the QR code?
Yes. The code points at the form's address, not its contents — add, remove or reword questions freely and every printed code still works.
Where do the responses go?
Into the form's Responses tab, and into a Google Sheet if you've linked one. The QR code changes nothing about how responses are collected.
How do I track how many people scanned versus opened the form?
Add UTM tags to the form link and check your analytics, or compare scan-driven response counts over time. See tracking QR scans for free.
Can I use this for Microsoft Forms or Typeform too?
Yes. Any form with a public link works — paste its URL instead. The steps are identical.
